PD Catheter Clinical Programme – Advancing Single-Surgeon Peritoneal Dialysis Catheter Insertion
The PD Catheter Project explores the use of the FreeHand® robotic camera control system during laparoscopic peritoneal dialysis (PD) catheter insertion, with the aim of supporting a true single-surgeon approach while maintaining procedural efficiency and consistency. The project is led by Mr Rajesh Sivaprakasam, Consultant Transplant and Renal Access Surgeon, and is being conducted within Barts Health NHS Trust, including work associated with The Royal London Hospital, one of the UK’s leading centres for renal failure surgery, dialysis access and transplantation.
The FreeHand Perspective
Evidence generated through the project to date suggests that experienced laparoscopic surgeons are able to integrate FreeHand into routine PD catheter procedures without significant disruption to workflow. Early findings indicate that operative performance remains consistent throughout adoption, supporting the view that FreeHand can be incorporated into established surgical practice with minimal adjustment.
The project also reinforces the broader FreeHand® vision of empowering surgeons with direct control of the laparoscopic view, reducing reliance on camera assistants and helping to standardise the surgical environment. Current observations suggest that the technology can deliver outcomes comparable to conventional laparoscopic approaches while supporting greater autonomy within the operating theatre.
As the programme continues to evolve, the findings further support the role of FreeHand® as a practical and accessible technology for centres seeking to improve efficiency without compromising surgical quality or patient care.

Dr Rajesh Sivaprakasam, MBBS, MRCS, FRCS, is a Consultant Transplant, Dialysis Access and General Surgeon at Barts Health NHS Trust and a leading advocate for the adoption of robotic and digital technologies in surgery.
Based at The Royal London Hospital, Mr Sivaprakasam specialises in renal transplantation, dialysis access surgery and advanced minimally invasive techniques. Alongside his clinical work, he has contributed to numerous research and technology initiatives. focused on improving patient outcomes and modernising healthcare delivery. He is also involved in the development of digital health solutions, artificial intelligence applications and robotic surgery programmes, bringing together clinical expertise with a strong focus on innovation and education.
